The estradiol patch delivers bioidentical 17-beta estradiol through the skin at a steady rate. Because it bypasses first-pass hepatic metabolism, it avoids the clotting-factor increase that oral estrogen causes.

Is the patch safer than oral HRT? +

For women with clotting or cardiovascular risk factors, yes. Transdermal estradiol bypasses the liver and does not raise liver-produced clotting factors, so it carries a lower risk of venous thromboembolism and stroke than oral estrogen. The Menopause Society's 2022 position statement specifically recommends transdermal delivery for women with elevated VTE risk.

How often do you change the patch? +

Depends on the brand. Most patches (Climara, Vivelle-Dot, Minivelle) are changed once or twice weekly. Combination patches containing both estradiol and a progestin (CombiPatch) are twice weekly.

Does the patch fall off in the shower or pool? +

Modern patches are designed to stay on through normal bathing and swimming. Adhesion varies by brand and skin. Rotating application sites (lower abdomen or upper buttocks — not breasts) helps.
